Creating a New Form in Mirata

Create a New Form

  1. Navigate to the Designer URL in your browser (Google Chrome is the preferred browser).
  2. Click Create.
  3. Click Form.

  4. Select Blank to create a new form from scratch. Select From Existing to choose an existing form to create a copy. 
  5. Select a Form Type. Only users with access to the selected Form Type will be able to use the form. Form Types are managed in the Mirata Admin Tool. Each form must be associated with one and only one Form Type.
  6. Type a Name for your form. Every form needs a unique Name.
  7. Type a Description for your form so others know the purpose of this form.
  8. The ID will be automatically generated for you.
  9. Click “OK”.
  10. Now, your form is created and you’re reading to start dragging on fields. A workflow is automatically created and coupled with the new form.


Open an Existing Form

  1. Navigate to the Designer URL in your browser (Google Chrome is the preferred browser).
  2. In the Open Recents section, the 5 most recently opened forms will display for quick access. If you’re looking for one of these forms, just click on its tile to open that form.
  3. The Name, date last Modified, user last Updated By, and Version number will display for all forms, which you have access to, in a scrolling list.
  4. Use the search bar to search by form name. As you type into the search bar, table #3 will filter to show forms with matching names. Once you find the form you want to edit, simply click it to open that form.


Go Back / Open a Different Form

  1. After opening a form, click the home button in the top right corner to go back to the form selection page.
